  • Name of applicant

  • Name of principal contractor (if different to applicant)

  • Proposed area of ground disturbance (this can just be a sketch)

If applicable

  • Indicative location of any proposed soil stockpiles

  • Indicative location of any site compounds or offices

  • Indicative location of any existing or proposed retaining walls

  • Soil type (if known)

  • Location of any existing or proposed concentrated flow paths including channels, drains, creeks etc

  • Any additional relevant information or comments
